$130 for the one hour initial visit

– no call out fee for initial visit
$40 call out fee outside of Calgary city limits per location
– fee shared among owners at the same location
**no call out fee if treating 3 or more animals at the same location**

Please take a few moments to complete the following forms before your appointment. We do need the consent form from your vet so please ask them to fill that one out. These can be emailed to peakperformchiro@gmail.com or brought with you to your appointment.

APPOINTMENTS
If you would like to make an appointment, please contact Peak Performance Animal Chiropractic by phone or email. All new patients must have a written referral or a completed consent form from your veterinarian. This must be done before any treatment can be given.
FINANCIAL
We currently accept only cash, eTransfer or cheque. Payment is due at the time of service.
CANCELLATION
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.

For the first time in Dr. Sarah’s equine practice, the high cost of fuel has forced a tough financial decision. Effective January 1, 2022, you will notice a slight increase in Dr. Sarah’s equine chiropractic fees. Both initial and regular treatments will increase by $10.
