EQUINE PATIENTS
Mobile Practice
* Please note – Effective May 2024, Dr. Sarah is unable to accept new horse patients at new locations. If you have a new horse at one of her current locations, please reach out to her regarding a new patient.
FEES
$100 per animal following initial visit
$130 for the one hour initial visit
– includes an in-depth history, exam, gait analysis, and treatment
– no call out fee for initial visit
$40 call out fee outside of Calgary city limits per location
– fee shared among owners at the same location
**no call out fee if treating 3 or more animals at the same location**
If Dr. Sarah is already at your barn, please ask about treatments for your barn dog!

BEFORE YOUR APPOINTMENT: Please take a few moments to complete the following forms before your appointment. We do need the consent form from your vet so please ask them to fill that one out. These forms can be emailed to: peakperformchiro@protonmail.com or faxed to 403.995.4394
PPAC Vet Consent – 2024 Either this form OR a written referral MUST be completed by your veterinarian prior to the first treatment**
APPOINTMENTS
If you would like to make an appointment, please contact Peak Performance Animal Chiropractic by phone or email. All new canine patients must have a written referral or a completed consent form from your veterinarian. This must be done before any treatment can be given.
FINANCIAL
Equine patients: cash, eTransfer, or cheque
Canine patients (in clinic): credit card, debit card, cash, eTransfer, or cheque
Payment is due at the time of service.
CANCELLATION
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
